The Centers for Medicare & Medicaid Services' (CMS) Money Follows the Person (MFP) Initiative helps states rebalance their long-term care systems to transition people with Medicaid from institutions to the community. MFP is being implemented in 43 states and the District of Columbia and to date has helped transition more than 22,000 people with disabilities to community living.

This year's annual MFP Project Directors Meeting, Building Partnerships for Sustainable Community Living, was held November 14-16, 2012 in Baltimore, MD. The meeting was organized around seven tracks which included: Community Inclusion; Data; Housing; Managed Care; Program Operations; Rebalancing; and Systems Change Opportunities.

TAC was tasked with developing the Housing Track for the meeting. TAC Senior Associates Emily Cooper and Lisa Sloane presented and/or moderated all of the housing sessions which covered a variety of topics including Housing 101, using tools such as housing locators and databases, state and local housing and service agency collaborations, and developing strategic housing plans.
