Successful MFP Transition Highlighted in Illinois

A recently published article highlights the experience of a 27 year old disabled Money Follows the Person (MFP) Demonstration program participant who successfully transitioned from a nursing home to his own accessible apartment in Oak Park, IL, thanks to a collaboration that combines Non-Elderly Disabled Category 2 (NED2) vouchers from HUD with Medicaid-funded supports. The state's MFP Program is administered by the Illinois Department of Healthcare and Family Services, which under an Olmstead Settlement Agreement ( Colbert v. Quinn) will transition 1,100 individuals from nursing homes over the next 2 ½ years, offering community housing opportunities with support services to residents receiving Medicaid in 185 nursing homes in Cook County, IL.
